Service Description

Paperwork Completion by a Private Pay UAI Certified, LPN to include: • Medical Release • DDNR education •The assistance of ALF required History and Physical examination via a medical professional • TB forms • Completion of paperwork from the ALF of your choice • Obtain copies of Insurance and Prescription cards, Driver’s License/State ID, Military ID Card, and Social Security Card as needed by VA ALF. • Set up a medical appointment(s) to complete the required paperwork • Set up of transportation needs, if applicable, to medical appointment(s) for completion of history and physical examination • Distribution of necessary medical forms for ALF placement
